Chronic Disease Annual Reviews

This is an annual nurse led review of a patient’s chronic problems the include:

  • COPD/Asthma
  • Heart disease
  • Hypertension (high blood pressure)

This will be done on the month of the patient’s birthday.

You should receive a letter the month before your birthday to advise if you need a blood test and invite you to book an appointment with one of our practice nurses.

In addition patients suffering hypothyroidism will be sent out a blood test once a year on the month of their birthday

Other Chronic Conditions not covered by the Annual Reviews

These are listed below and if you have not discussed these with a doctor or nurse in the past year then please book an appointment.

  • Rheumatoid Arthritis
  • Mental Health Illness
  • Learning Disability
  • Dementia

Childhood Immunisations

There are carried out by the practice nurse in a routine appointment if you are unsure if your child is due a vaccine then please look at the NHS vaccinations and when to have them page. 

This is for children of all ages who require immunisations. 

If your child is unwell then we can re-arrange your appointment for a later date when you think it is likely that your child will have recovered from their illness

Diabetes

Diabetes clinics are led by our practice nurses.

This would include a foot check, blood pressure check and dietary advice.

You may also be referred for blood tests, chiropodist or to dietician.

Depending on how well the condition is controlled you are usually seen twice a year.

You are usually sent a letter for an annual appointment for diabetic eye screening – this is not generated by the practice.

 

Contraception Clinic

These are run by Dr. Hughes (Monday afternoon) and Dr. Karir (Friday afternoon).

They are for female contraceptive procedures:

  • coil fit / change / check / removal – all for ‘T-Safe’ and ‘Mirena’ coils;
  • implant fit / change / removal

A telephone appointment will be required before any of the above procedures will be carried out.

If you would like to commence on the Depo contraceptive injection, please make an appointment with Nurse Justine. If you require a top-up of your Depo injection, please make an appointment with either Assistant Practitioner Helen or any of our Nurses. Neither of these appointments require a GP appointment, unless you are under 16 AND wish to commence the Depo injection.

If you would like to commence on the contraceptive pill, please make an appointment with Nurse Justine. If you require an additional contraceptive pill prescription, please make an appointment with either Assistant Practitioner Helen or any of our Nurses. Neither of these appointments require a GP appointment, unless you are under 16.

If none of the appointments we have available are convenient, Riversley Park Clinic offers a Contraceptive Clinic by appointment only. Call 02476 378601 for further information. For under 25s, contraceptive services are also available at Hatters Space Community Centre (02476 651418) and Camp Hill Community Centre (02476 390005).

NHS Health checks

The NHS Health Check is a health check-up for adults in England aged 40 to 74. It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As you get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.

Even if you currently feel well, it is still worth taking advantage of our FREE NHS Health Check as this can lower the chances of you developing the aforementioned conditions in the future.

A free NHS Health Check performed by Bedworth Health Centre should take no more than 20 minutes and can be performed by one of the Nursing team.

They will ask you some simple questions about your family history and current lifestyle and then will take some basic observations such as height, weight, blood pressure and cholesterol.

They will then discuss what your results mean and you will be given personalised advice on how to lower your risk of developing one of the aforementioned conditions and to maintain a healthy lifestyle.

Travel Vaccinations

If you're planning to travel outside the UK, you may need to be vaccinated against some of the serious diseases found in other parts of the world.
